PAEDIATRIC SURGERY

All surgical procedures undertaken by qualified Pediatric surgeons only.

All patients undergoing surgical procedure (either routine or emergency) get assessment done preoperatively and provisional diagnosis is documented.

Precautions are taken to prevent any adverse event. All patients undergoing surgical procedure are identified through UHID number and name and preoperative checklist verified by the Pre-OP sister-in charge and OT in-charge. Site of surgery on patient confirmed by surgeon, anaesthetist and sister prior to surgery.

The surgeon performing the surgery or a credentialed doctor from the team does the pre-operative assessment.

All patients planned for routine surgical procedure get their pre-operative investigations and physical fitness done in advance, prior to admission whenever possible.

All pre-operative consents taken prior to any procedure after proper explanation to patient/relative. A fresh consent is sought in case of change/ addition in operative procedure intra-operatively.

Post-operative notes are prepared by the surgeon which includes procedure performed, post-operative diagnosis, plan of care and status of the patients and documented prior to transfer out of patient from recovery area.

DISCHARGE

Patients are discharged with order from the primary consultant. However, a patient may request discharge against medical advice. All discharge instructions are documented by consultant/assigned staff in the patient’s medical record at the time of discharge.

A Discharge Summary is prepared.

The nurse shall be responsible for completing the discharge checklist and explaining the discharge process to patients.

Consultant/ Fellows/ Residents completes the discharge document and explains it to patient.

Patients requesting DAMA are explained the risks and consequences. Patient’s/ relatives’ signature is obtained in DAMA form in Inpatient Record
